Nuevo libro recuenta los tratamientos que estigmatizaron la psiquiatría
Ésta práctica ha sido detallada en "Breve historia de los tratamientos biológicos en la psiquiatría", del investigador peruano Santiago Stucchi.
Este psiquiatra, profesor e investigador de la universidad peruana Cayetano Heredia, ha publicado el libro, que es una revisión de los métodos empleados antes de la existencia de los psicofármacos.
Stucchi explicó que al hablar de tratamientos biológicos en la actualidad, se refiere principalmente a terapias y métodos que, en su mayoría ya no se usan, "pero tienen un pasado no muy lejano". "Es todo lo que implica una acción directa en el cuerpo, a diferencia de las terapias psicológicas, que actúan a través de la palabra, de la conversación", acotó.
Entre aquellos figuran la lobotomía, las convulsiones por sustancias, la insulinoterapia y la malarioterapia (provocación deliberada de fiebre palúdica para tratar algunas enfermedades nerviosas), de principios del siglo XX.
El libro también hace una revisión de los principales métodos no psicológicos utilizados "desde los tiempos dominados por las concepciones mágico-religiosas". Se mencionan desde los hospitales-prisiones, las esterilizaciones forzadas y los experimentos no autorizados, hasta la psicocirugía y el electrochoque, que han sido "exaltados sin piedad por los entusiastas partidarios de la anti-psiquiatría".
Stucchi explicó que muchos de estos métodos fueron propios de una etapa anterior a la psiquiatría, que recién apareció como tal en el siglo XVIII, con el médico francés Philippe Pinel. "Hasta antes de eso se hablaba de la alienación, y los médicos que ahora se llaman psiquiatras eran los alienistas, no era exactamente lo mismo, pero es el antecedente más próximo", sostuvo.
En el siglo XVII se dio "el gran encierro" -como lo denominó el francés Michel Foucoult-, en los grandes hospitales prisiones "de todos los elementos marginales de la sociedad", incluidos los "insanos", prostitutas, vagos y delincuentes.
"No es que se encierre a las personas con problemas mentales para estudiarlos psiquiátricamente, sino que la psiquiatría surge poco a poco para que las personas encargadas del cuidado de estos individuos puedan manejarlos", refirió. Esto permitió que métodos como los grilletes, el agua fría y las sillas giratorias vayan cediendo paso "poco a poco" a tratamientos cada vez más científicos a lo largo del siglo XIX.
Perú en el contexto
Stucchi remarcó que "Perú no fue ajeno para nada a la realidad internacional" y reveló que en Lima se aplicó el encierro en dos grandes hospitales, conocidos en esa época como "loquerías": la de Santa Ana, para las mujeres, y la de San Andrés, para los varones. Luego se abrió el Hospicio de la Misericordia, en 1859, y se construyó un asilo colonia en la zona de La Magdalena, que funciona desde 1918 y es el actual Hospital Víctor Larco Herrera.
En el país también se utilizaron las terapias de choque, entre ellas la electroconvulsiva, conocida como electrochoque, "que de los tratamientos pre-farmarcológicos es el único que sobrevive hasta la actualidad".
El investigador, cuyo libro ha sido publicado por el Centro Editorial de la Universidad Cayetano Heredia, aceptó que la aplicación de los métodos biológicos influyó "indudablemente" en la leyenda negra que arrastró la psiquiatría a lo largo de su historia. Sin embargo, aclaró que aunque "todavía existe la asociación entre psiquiatra y paciente que está fuera de sus cabales", esta ciencia no sólo trata la esquizofrenia, un problema mental que sólo padece el uno por ciento de la población.
La psiquiatría moderna también es empleada para enfrentar desde la depresión hasta la bulimia o la anorexia, enfermedades que, según aclaró Stucchi, "siempre han existido, pero lo que ha cambiado son las denominaciones y los diagnósticos".
